Angels Post-Adoption Support Program
The Post-Adoption Support Program offers personalized services to help adoptive families support their children’s well-being at home, in school, and in the community. Our team provides in-home support, school advocacy, behavioral strategies, and connections to community resources.
We partner with families to strengthen attachment, address challenges, and promote long-term stability. Services are trauma-informed, flexible, and tailored to each family's unique needs. For eligible families, we also offer Enhanced Care Management (ECM)—a Medi-Cal program that coordinates physical, behavioral, and social services to meet the holistic needs of children and caregivers.
Angels delivers ECM in partnership with Medi-Cal health plans including Molina, Kaiser, and Blue Shield.
Intended Participants
This program is designed for families who have finalized an adoption through the child welfare system and are caring for a child under age 18 who receives Adoption Assistance Program (AAP) benefits. The program also serves youth under age 21 with a recent history of foster care.
Eligibility
Children or youth must meet one or more of the following criteria:
Under age 18 and receiving or eligible for Adoption Assistance Program (AAP)
Under age 21 with a history of foster care within the last 12 months
Service Details
Cost: Free for eligible families through ECM
Health Plans: Services available through Medi-Cal plans: Molina, Kaiser, and Blue Shield
Enrollment Support: If the child or youth is not yet enrolled in a Medi-Cal health plan, our team can assist with enrollment or switching providers
Locations: Services are delivered in the home, in the community, or at our office in Kearny Mesa
